HHS lists covered entities reporting breaches of PHI affecting more than 500 individuals
OCR posted on its Web site February 22 a list of the covered entities that have reported breaches of unsecured PHI affecting more than 500 individuals. HITECH requires OCR to make public any breaches affecting 500 or more individuals.
OCR, which enforces the HIPAA privacy and security rules for HHS, plans to continue updating the page as it receives new reports of breaches of unsecured PHI.
